A recent high school graduate awaiting her fate in the Neonatal unit of a hospital (With some critical thinking we can infer that she is meeting her baby or having her baby) is not a scene we expect to see and it aims to point out how awful it is that a teenager would be forced to carry a child to term and not have autonomy to choose not to. The cap and gown indicate how, as a graduate, she would expect to be entering college or an adult life and career but she is forced into being a parent and now has a different future than she may have envisioned for herself.
