Family Academy Sign-Up and Consent Form

Purpose

Professors in the Department of Psychology at Western Washington University maintain a list of families who are interested in participating in the research conducted by our faculty and students. By filling out this form, you are consenting to be included in a list of families interested in participating in our research studies. Adding yourself to this list does not mean that you are agreeing to participate in a research study. Being on this list means that you are willing to be contacted by Western Washington University Psychology Department researchers with information about studies in which you or your child(ren) are eligible to participate.

Risks and Benefits

There are no risks to becoming a part of this list. The benefits include the opportunity to participate in studies that you and your children might find interesting and rewarding, as well as contributing to our understanding of families and child development.

Data Security and Confidentiality

Information in this list will be used only by Western Washington University Psychology Department researchers for the reasons stated, and it will be stored securely. This list is stored on password protected computers and only used by researchers with approved access for the stated purposes of the research study.
